In our experience, approximately 15% of patients respond well to conservative treatment alone, including hair control, hygiene optimisation, local care and lifestyle modification. Patients are offered procedural intervention only when there is a clear clinical need and potential benefit.

A major differentiator of our practice is the routine use of ultrasound imaging before, during and after treatment. Before treatment, ultrasound helps map the sinus tracts accurately, identify hidden branches, and detect retained hairs within scar tissue from previous inflammation. During minimally invasive procedures, ultrasound guidance and the use of a guidewire under ultrasound vision help ensure accurate access without creating false passages or new tracts.

Following treatment, ultrasound assessment enables us to monitor healing in a controlled, predictable manner, rather than relying solely on patient-reported symptoms. This reduces uncertainty and supports more effective long-term management.

Our discharge criteria are objective and measurable, and patients are discharged only when:

  1. The skin over the sinus opening has fully closed and has a thickness of 0.5 mm or more

  2. There is no retained hair within the tract

  3. Treated areas demonstrate fibrosis and stable healing on ultrasound

This approach helps reduce premature discharge, recurrence, persistent drainage and long-term discomfort.
